Quality Improvement

The Community Clinic Consortium is committed to supporting our health center members provide the highest quality care possible – with a focus on putting the patient first. We are engaged with our members in various high-impact initiatives such as achieving health home recognition or meeting performance targets in health plan pay-for-performance programs.

Through group learning opportunities and one-on-one coaching, we offer support in:

  • Assessing readiness for quality transformation
  • Establishing a plan of action, or ‘roadmap’ for improvement
  • Identifying goals and metrics and monitoring performance through use of tools such as dashboards
  • Content specific areas such as identifying and strengthening the care team and fostering patient centered interactions
